City Blames Workers for Elevator Accident that took New York Woman’s Life

Employees at a Madison Avenue office building were shocked when an elevator malfunctioned on December 14th and took the life of advertising executive, Suzanne Hart.

An investigation onto the accident was launched and after two months, the people looking into the matter have determined that the accident was caused by a worker’s negligence.

Hart became trapped in between the elevator and shaft when she attempted to get on with two other passengers. As she stepped inside the elevator it began to shoot upwards, causing her to fall forward with her legs hanging out as it ascended. Hart was crushed and died on the scene.

Normally, an elevator has a safety device, which prevents the car from moving if the doors are open, but that safety feature had been disabled so that en employee from Transel Elevator Inc. could service the elevator. 

Investigators concluded that the Transel employee failed to put the safety system back online when Hart stepped into the elevator. Transel asserted at the time that the elevator malfunction was not caused by any of their employees.
