San Francisco General Hospital Pilot Study Demonstrates Significant Improvement in Readmission Rates with Meducation®

Morrisville, NC – November 4, 2015 – Polyglot Systems announced today that a pilot study conducted at San Francisco General Hospital (SFGH), using the company’s Meducation® solution, has demonstrated significant improvement in patients’ 30-day hospital readmission rates.

Meducation creates simplified medication instructions at lower reading levels, in more than 20 languages, with larger font sizes, pictograms and videos, to make it easier for patients to understand how to take their medications. Meducation is a cloud-based software application which allows for easy integration with hospital or pharmacy systems.

The SFGH pilot study focused on 70 high-risk patients admitted to the hospital. Twenty-six percent of patients receiving standard medication instructions and counseling at discharge were readmitted to the hospital within 30 days. However, patients who received pharmacist counseling using medication instructions from Meducation®, returned to the hospital only 8% of the time — a 70% improvement.

“Poor medication adherence is a major contributor to unnecessary hospitalizations,” said David Smith, PharmD, who led the SFGH study. “As a public hospital, providing medication instructions in the patient’s native language with easy-to-understand formats is a key component to empowering our patients and lowering hospital readmission.  Meducation is a powerful tool to accomplish this.”
